Proviso Township offers FREE rides to senior citizens. Proviso Township offers a free Senior Ride Program for residents at least 60 years of age who need transportation to and from medical facilities and other qualifying non-medical locations within the Township. All participants must reside in Proviso Township. The Senior Ride Program is a curb to curb service that requires all participants to be able to walk unassisted to and from the Township vehicle.

Please register with the Senior Services Department by filling out the form below, scanning and attaching a proof of age, address, and the signed Client Agreement Form. You can also register in-person at the Township office in Hillside or call us at 708-449-4307 and we will explain how to register via mail, email, or FAX.
